Completing Outcome Measures

Your clinician may ask you to complete a questionnaire, or a "standardized outcome measure" as formally referred to.  This outcome measure is a tool for your clinician to get an understanding of the status of your condition via a numerical score.  You may only be given this outcome measure once, however you could be asked to complete one several times to help track your progress.  

If your clinician gives you an outcome measure, here's how you access it:

1.  You will receive an email from your clinician letting you know to complete a questionnaire.

2.  In the body of the email will be a link - it should look like this:

Click on the "Complete Questionnaire" link.  

3.  You will be taken to the Client Portal.  If this is your first time in the client portal you will be prompted to create a Client Portal account.  If you need help with that, click hereIf it's not your first time, log in as previously.  

4.  Select the answer to each question that best suits you.  Once done, hit submit and your clinician will receive it.  That's it!
